Abuse, Neglect & Exploitation Reporting

Immediate reporting options and protections when abuse, neglect, financial exploitation, or a safety threat is suspected.

Urgent action

If someone is in immediate danger or needs urgent medical help, call 911. Do not use a website form for an emergency.

Report directly to Illinois DHS OIG

The Illinois Department of Human Services Office of Inspector General investigates allegations of abuse, neglect, and financial exploitation involving adults with disabilities in CILA and other covered settings.

  • DHS OIG Hotline: 1-800-368-1463
  • Report suspected abuse or neglect promptly; proof is not required before making a report
  • Criminal conduct or an immediate threat should also be reported to local law enforcement

Tell First Zion

Also notify a First Zion supervisor or executive leader so immediate protective steps can be taken. An internal notification does not replace a required OIG or law-enforcement report and should never delay an external report.

Protected reporting

First Zion prohibits retaliation, intimidation, interference, concealment, destruction of evidence, or obstruction of an investigation. Employees must follow mandatory reporting duties and cooperate honestly with authorized investigations.

What may be reported

  • Physical, sexual, or mental abuse
  • Neglect or failure to provide required care and supervision
  • Financial exploitation, theft, coercion, or misuse of property
  • Retaliation, intimidation, concealment, or interference with reporting
  • Any unexplained injury, unsafe condition, or conduct that reasonably raises concern
