How service information is protected, appropriately shared, accessed, and handled under applicable confidentiality requirements.

Confidential service information
Personal information and communications concerning developmental-disability services are handled in accordance with the Illinois Mental Health and Developmental Disabilities Confidentiality Act and other applicable requirements. Information is accessed only for authorized purposes and disclosed only when permitted or required by law.
Consent and access
Individuals and authorized representatives may request access to records and information about disclosure rights. When authorization is required, First Zion uses a valid written authorization describing what may be shared, with whom, and for what purpose.
HIPAA status
HIPAA applies only when an organization meets the legal definition of a covered entity or business associate. First Zion will provide any legally required Notice of Privacy Practices directly to affected individuals. This page does not claim or disclaim covered-entity status and does not replace an individualized notice.
Secure communication
Do not send medical records, Social Security numbers, financial account information, or other highly sensitive material through general website forms or the enquiry assistant. Ask staff for an approved secure method.
