How to raise a concern
A concern may be shared verbally or in writing with any staff member, supervisor, program leader, or executive leader. A person may ask someone they trust to help, request communication assistance, or make an anonymous complaint where practical.

What happens next
First Zion will route the concern to an appropriate person, review the available information, protect confidentiality as permitted by law, communicate with the person raising the issue when contact information is available, and document corrective actions when warranted.
Appeals and independent review
Individuals and guardians may present grievances and appeal adverse service decisions through the authorized CILA agency representative. Applicable Medicaid HCBS notices, appeals, fair hearings, and administrative review rights remain available. Asking for review does not waive any external complaint right.
No retaliation
No person will be threatened, punished, denied services, have services reduced, or experience adverse employment treatment for making a good-faith complaint, participating in a review, contacting an advocate, or reporting a suspected violation.
